First Visit:

Thank you for choosing our office for your family’s medical care needs.

If at all possible, please arrive at least 15 minutes early to complete registration forms.

Also, please bring your insurance card and a photo ID. If you are bringing your child, please bring his/her immunization records as well.

We want you to feel at ease when you visit. Our friendly, professional staff is always available to assist you with any questions/concerns you may have.

Our physicians and nurses are experienced and aware of recent advancements in family medicine. But good medical care begins with open communication. We promise to speak candidly with you about any symptoms, our diagnosis, alternative treatments that may exist, and our recommendations. In return, we hope you will ask any questions you may have before, during and after your treatment with us.

Established Patients:

For each visit, please try to arrive at least 15 minutes early. This will allow you to verify your patient information and make any necessary updates.

Certain types of visit require the completion of specific forms. For instance, adult physical exams include the physical questionaire. Children's sports, camp, and kindergarten physicals also require completion of a questionnaire. Early arrival should allow time for completion of any needed paperwork.

If you are having your cholesterol (lipids) checked, please make a morning appointment. These tests require fasting. Fasting means no food or drink (water is ok) for at least ten hours prior to your lab test. We find this to work better for most patients early in the day.

If your appointment is for an annual physical, an adoption physical, a camp or school physical or travel consultation, please bring with you a record of your previous vaccinations.
